Registered Nurses, Grade 2, Year 5 plus, Permanent Positions

(up to 76 hrs per fortnight)

48 to 64 hrs per fortnight

We are seeking highly motivated Registered Nurses with the skills and qualities to assist in the organization and coordination of delivery of care in St Lukes Hospice Unit and St Josephs Independence Maintenance Unit.

The successful applicant will demonstrate:
- A patient and family-centred approach to care delivery.
- Competency in clinical assessment, consultation, liaison and care provision along a continuum of care.
- A strong commitment to customer service and risk management.
- A desire to develop their leadership and management skills in a supportive environment and act as a role model for nursing staff.
- A commitment to their own learning and an ability to facilitate nursing staff professional development.

Essential Qualifications:
- Registered Nurse, AHPRA / Nursing and Midwifery Board of Australia

Experience in Palliative or Neurological Care would be desirable

We are seeking a highly motivated individual with a demonstrated interest in person-centred care and care planning to join our Statewide Progressive Neurological Disease (PND) Service. The Statewide PND Service is a specialist provider of interdisciplinary assessment and management services for people with complex needs due to a diagnosed PND such as Motor Neurone Disease, Huntingtons Disease, Muscular Dystrophy, Multiple Sclerosis, Parkinsons Syndromes and others.

Grade 3 Nurses provide comprehensive care and management advice to clients diagnosed with a PND and their families. Nurses facilitate care planning via phone triage, needs analysis, appointment scheduling, shared care planning and liaison with members of the interdisciplinary and research teams. Nurses within the service carry a case load and work closely with Neurologists to provide specialist care. This role also involves provision of education and secondary consultation advice to external health care providers.

The successful applicant will demonstrate:
- Highly developed interpersonal skills and professionalism
- The ability to coordinate client care
- The ability to identify and respond to the changing needs of individuals with a PND
- The ability to work collaboratively and effectively within a multidisciplinary team

Prior clinical experience in working with individuals with Progressive Neurological Conditions or chronic disability is desirable, but not essential. Applicants must be eligible for on-going entitlement to work in Australia and hold a current and valid Victorian Driver's Licence
