Home and Community Care (HACC)

The Home and Community Care program aims to fund a range of basic support services to enable eligible clients to continue living at home in the community, as well as enhancing their quality of life.  Services can include:

  • Personal care
  • Domestic assistance
  • Meal preparation
  • Social Support
  • Respite care
  • Transport
  • Home maintenance

Referrals can be provided through various avenues such as your doctor, community health centres, councils, or in some cases friends, family or self referrals.

After a referral is received, the service coordinator will make an appointment to see you to talk about services available and assess your eligibility. If eligible, a service plan will be formulated and service put in place.
