Extended Aged Care Packages Dementia (EACHD)

An Extended Aged Care at Home Dementia Package is a clinical care and support package of services to help older people with dementia and associated behavioural conditions to remain at home, as an alternative to moving into a residential facility. They are flexible to adapt to the changing needs of the client.  Calvary Silver Circle will work with you and/or your representative to design a package of services to meet your individual requirements, preferences and circumstances. Services can include:

• Clinical Care ie medications, wound management, continence advice and other specialised nursing needs
• Access to allied health and related services
• Personal hygiene, dressing and grooming
• Meal preparation
• Laundry and home help with domestic tasks
• Shopping, banking and social outings
• Transport
• Home safety and security

To receive an Extended Aged Care At Home Dementia Package, you must be assessed by an Aged Care Assessment Team (ACAT) as requiring high level care and experience behaviours and psychological symptoms associated with dementia that is significantly impacting upon your ability to live independently in the community. The ACAT assessments are free of charge. Calvary Silver Circle may be one of the organisations which will be advised of your eligibility for a care package. You will then be offered a package as soon as one becomes available.
